benefits here.DETAILED JOB DESCRIPTIONCollaborate with other
members of the ALS/MDA/Neuromuscular clinical teams and ensure that
the clinical program facilitates well-coordinated, timely,
compassionate, multidisciplinary care.Acts as the initial and
ongoing point of contact for patients as they enter the system for
work-up, diagnosis, and treatment. Coordinate appointments,
including OR, diagnostic testing, follow-up needs, and follow-up on
diagnostic results.Obtain patient information and orders for
necessary testing and lab work.Promote and support opportunities
for participation in clinical trials and communicate those patients
to the appropriate teams.Provide education about diseases and
diagnoses, treatment options, treatment side effects, preventative
and health maintenance behaviors, procedures, and team member roles
to patients, families, staff, and the community.Contact patients
prior to upcoming multi-disciplinary or other neurology visits to
determine pending needs and whether the previous treatment plan has
been completed. Gather cases and documentation for presentation at
conferences. Facilitate needed care in the interim. Document plan
of care in medical record, and provide updated plan to other
services, physicians, and the patient.Responsible for tracking
program quality indicators, including keeping accurate patient
census and diagnosis data. Provide data to the Director of
ALSA/MDA/Neuromuscular Clinics and multi-disciplinary team.Assess
and evaluate patients for healthcare referral needs, including, but
not limited to, social, dietary, psychological, and financial
services. Function as a liaison between the multidisciplinary team
and physicians, departments, and other referring medical
providers.May complete unit/department-specific duties as outlined
in department documents.REQUIRED QUALIFICATIONSMissouri Board of
Nursing RN or Nurse Licensure Compact multi-state. When primary
state of residency changes, compact state RNs must apply under new
state of residency within thirty (30) days.Three (3) years of RN
experience.Three (3) years of related professional experience,
including direct care, case management and leadership or management
experience.PREFERRED QUALIFICATIONSBachelor's or master's degree in
nursing.Research experience.Additional license/certification
requirements as determined by the hiring department.PHYSICAL
